Description :

The problem is the oil ring Is placed incorrectly
so that's causing the oil ring to move out of its place and the result from that's
is friction with the shaft. Due to erosion, so small pieces of material fall to
the lubricating oil that causes a change of its color and increases the
temperature.

Proposed/Recommended
Action:
First, new grooving is done for the oil ring. New
oil ring is replaced and put it in the correct position.

Comments (if any):
The action that’s will solve the problem so that’s will help to prevent the oil ring to move out and that means no more friction and temperature. More monitoring for the lubricant will be done.

Fuel oil passing through the in-service filter from the
standby filter. Causing failure to start for the gas turbine while some fuel of
the standby filter goes inside (actually crude oil enter the fuel oil line at
start-up period}.

Action:
Fix the issue.
· The action was taken by
Contractor to rectify Defect/Fault:
After testing the
valve by filling one filter with fuel oil and emptying the other filter with
directing the valve to the empty filter they observe the passing issue. For
that the action is to replace the valve with a new valve.

Clean the filters (by diesel) after every
operation.
